Incidents

An incident records something that went wrong: a near miss, damage, a spill, an injury, a security event.

What an incident holds

Beyond the description and severity, an incident can carry:

• the locations it affected
• the vessels involved
• witnesses
• corrective actions — what was decided, and whether it has been done

Each of these is a record in its own right rather than free text, so an incident affecting three berths can be found from any of them.

Status

An incident moves from reported through investigation to closed. Closing an incident does not delete anything: the account of what happened, and of what was done about it, is exactly the thing an incident record exists to preserve.

Who can see them

Incidents follow the same access rules as every other operational record — they are visible to members of the account, scoped to the ports those members can reach. Their edit history is visible to account owners.
